Nascentia Health is seeking an Registered Nurse Case Manager to join our Adult Homecare Team serving Onondaga, Oneida, Oswego, Jefferson and Cayuga Counties. This full-time role involves in-home assessments, care coordination, and patient education to promote independence and quality of life.
Responsibilities include delivering high-quality nursing care at patients' homes, coordinating with an interdisciplinary team, documenting accurately, and supervising home health aides.
