Job description
Phoenix Home Care and Hospice in St. Louis, MO is seeking a Home Health RN Case Manager to join our team.
The role offers a base salary of $78,000-$88,000 with a $10,000 sign-on bonus, a Monday-Friday schedule, and a pay-per-visit model that rewards the number of visits completed. You will manage a caseload, supervise LPN visits, coordinate with Physical, Occupational, and Speech Therapy teams, and conduct assessments to build care plans aligned with physician orders.
