Job description
Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager in Victoria, TX to coordinate all aspects of the patient’s home care. You will perform admission visits, assess eligibility for services, and develop the plan of care in collaboration with medical teams.
The ideal candidate is a graduate of an accredited nursing program, holds an active RN license in Texas, and has CPR certification. Valid driver’s license and reliable transportation are required.
