Job description
Amedisys is seeking a part-time RN case manager to cover the Haven area in Shelton, CT. The role offers 20-30 hours per week with benefits and a focus on home health care, providing patient assessments and care coordination in patients' homes.
Responsibilities include developing plans of care, coordinating with the care team, and providing education to patients and staff. Prior RN experience and a valid CT RN license are required.
