Job description
Nascentia Health is seeking an experienced Registered Nurse to join our Adult Homecare Team as an RN Case Manager serving patients in Onondaga, Oneida, Oswego, Jefferson, and Cayuga Counties. You will deliver nursing care at the patient's home, coordinate with an interdisciplinary team, and develop individualized plans of care.
