Job description
Nascentia Health is seeking an RN Case Manager to deliver compassionate, in-home care for adults with chronic or complex medical needs in Onondaga, Oneida, Oswego, Jefferson, or Cayuga Counties. Expect a Monday–Friday schedule, no on-call, one weekend per month, and a comprehensive benefits package.
