Job description
Summa Health at Home is seeking an experienced Registered Nurse Case Manager to coordinate and lead home-based nursing care, partnering with physicians and an interdisciplinary team to deliver quality outcomes in the patient’s preferred environment.
You will assess, plan, and manage care, educate patients and families, and ensure safety while navigating state regulations and agency policies in a fast-paced home-care setting.
