Job description
Transitions At Home is seeking an experienced RN Case Manager to support our care team in coordinating patient-centered home health services. You will guide clinical and regulatory matters, train staff, review care plans, and coordinate services while counseling patients and families in the residence under supervision of the RN Clinical Manager.
