Job description
Transitions At Home for Southern Wisconsin is seeking an experienced RN Case Manager to join our team. This full-time position involves coordinating patient-centered care in the comfort of clients’ homes, providing clinical guidance, and executing care plans under the supervision of an RN Clinical Manager.
Applicants should have an active Wisconsin RN License and at least two years of nursing experience, with one year in Home Health preferred. We offer competitive wages of $34-$44 per hour, a company car, and various benefits to promote career advancement.
