Job description
Community Health Network, Inc. is seeking an experienced Home Care RN Case Manager to serve patients in the Waltham/Arlington/Lexington area. This full-time position involves managing a caseload in patients homes, from admission to discharge, with supervision of LPNs and Home Health Aides.
The role requires at least 1 year of Home Care RN Case Management experience with a Medicare-certified agency and offers a competitive pay package with mileage reimbursement.
