Nascentia Health in New York is seeking RN Case Managers to join our Adult Homecare Team serving Onondaga, Oneida, Oswego, Jefferson and Cayuga Counties.
Provide skilled in-home nursing, care coordination, patient education, and collaboration with the interdisciplinary team in a flexible, full-time schedule with generous benefits.
No on-call, one weekend per month, competitive salary and extensive benefits package.
