Job description
Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of home-based patient care. You will collaborate with interdisciplinary teams, perform admission visits, and develop individualized plans of care for each patient.
The role requires an active RN license, a valid driver’s license, CPR and Hepatitis consent/declination on file, and reliable transportation. This is a full-time position with comprehensive benefits and growth opportunities.
