Job description
Lifespark is seeking an experienced Home Health RN Case Manager in Minnesota to plan and direct home health services. You will lead the care team, complete start of care assessments, and develop individualized care plans for patients in their homes and communities.
The role offers regular business hours, field coordination with the care team, and mileage reimbursement. A valid RN license and Medicare home health experience are preferred.
