Job description
Angels Care Home Health in Texas seeks a Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will perform admission visits, determine eligibility for home health services, and develop the plan of care.
The role requires a nursing degree (Diploma/ASN/BSN), current RN license, valid driver's license, CPR and Hepatitis consent/declination, and at least one year of RN experience. Reliable transportation is essential.
