Job description
Bridge Home Health & Hospice is seeking a Registered Nurse to plan, organize and direct home care services within Sonoma County. The role emphasizes community health education and coordination of patient-centered care in home settings.
Responsibilities include assessments, care planning, medication administration, and educating patients and families. Must have CA RN licensure, CPR, and 2 years of licensed experience; a Bachelor’s is preferred with home health experience.
