Job description
Nascentia Health seeks a Homecare RN Case Manager to deliver skilled nursing in patients’ homes across Onondaga, Oneida, Oswego, Jefferson, or Cayuga Counties. You’ll coordinate care, educate patients and caregivers, and collaborate with an interdisciplinary team to support independence at home.
This full-time role offers a competitive salary and a 15,000 sign-on bonus, plus a comprehensive benefits package including tuition reimbursement and an on-site gym.
