Job description
Amedisys is seeking an experienced RN Case Manager to assess patients, collaborate with the care team, and develop individualized plans of care for home health patients in their own homes.
You will coordinate referrals, promote patient health through teaching, supervise LPNs and HHAs, complete timely documentation, and regularly communicate progress to the clinical manager. On-call duties and ongoing clinical development are also part of the role.
