Job description
All About You Collaborative Health Care Services, LLC is seeking a Full Time RN Case Manager to join our team serving Naugatuck and surrounding towns. The RN Case Manager will conduct initial evaluations, develop and implement care plans, coordinate services, and supervise aides in patients’ homes or Assisted Living Facilities.
