Job description
AccentCare is seeking an experienced RN Case Manager for Home Health in the Austin area. The role manages patient care plans from admission through discharge, coordinating RNs, LPNs, CHHAs, and other clinicians to ensure high-quality, patient-centered care.
It uses a per-point pay model and requires on-site availability in Bastrop, Del Valle, Lockhart, and Smithville coverage areas. Qualified candidates must be licensed Registered Nurses with at least one year of RN experience and CPR
