Job description
Interim HealthCare in Portsmouth, NH is seeking a Registered Nurse Case Manager to join our home health team. This full-time role offers the opportunity to build lasting relationships with patients and families while delivering skilled nursing in the patient’s home.
You will perform comprehensive nursing assessments, develop care plans, coordinate with physicians and therapists, educate families, and supervise the care team to ensure safe, independent living at home.
