Job description
Embrace Home Health is seeking a Registered Nurse Case Manager to provide skilled nursing care in patients' homes in Rock Hill, SC. The role coordinates treatment with a multidisciplinary team and families, ensuring high-quality, compassionate care while working independently with supervision as needed.
