Job description
RVNAhealth in Connecticut is seeking a compassionate Registered Nurse Case Manager to provide expert home health care across the Central Territory including Weston, Wilton, and Redding, CT. You will manage your own patient caseload and coordinate care with an interdisciplinary team to support recovery and independence.
You’ll perform Start of Care admissions, develop individualized plans of care, educate patients and caregivers, and ensure documentation and transitions meet state and Medicare
