Job description
Unifi Healthcare is seeking an RN Case Manager to ensure high-quality, patient-centered home care through development, implementation, and evaluation of individualized care plans. The role coordinates with physicians, families, and care teams, supervising clinical activities within policy guidelines.
The RN will manage care, ensure timely documentation, and support ongoing education and communication among staff, patients, and families to sustain outstanding service in the home setting.
