Job description
Alternate Solutions Home Care of Columbus is seeking an experienced Registered Nurse Case Manager (RNCM) to lead the assessment of patient condition and plan of care. You will coordinate multi-disciplinary care and ensure high-quality, cost-effective, billable care through direct care and supervision under physician direction.
You will educate patients and families, ensure safety, and maintain compliance with legal rules.
