Job description
AccentCare, Inc. is seeking an experienced RN Case Manager for Home Health in the Austin North Office.
This full-time role covers North Austin and surrounding areas, coordinating patient care plans from admission through discharge and guiding a care team to deliver quality services. The position requires an RN license, at least one year of RN experience, and CPR certification, with a schedule Monday–Friday and on-call as needed.
