Job description
AMEDISYS HOME HEALTH in Illinois is seeking a Registered Nurse Case Manager for full-time day shifts to cover Richmond, Spring Grove, McHenry, Antioch, Lake Villa, Round Lake and Grayslake. You will perform patient assessments, develop and implement plans of care, supervise LPNs and HHAs, complete timely documentation, and communicate progress to the clinical team.
