Job description
Ohio Living is seeking an RN Case Manager for Home Health to plan, coordinate, and deliver quality in-home nursing care in accordance with state and federal standards. This role emphasizes care planning, evaluation, and communication with an interdisciplinary team to meet patient needs.
The position requires an active Ohio RN license, at least one year in Medicare-certified home health, OASIS experience, and proficiency with EMR and Microsoft Office.
