Job description
Home Health Care, Inc. is seeking a Registered Nurse (RN) Case Manager for our home health team in Minnesota.
This full-time role features a Monday–Friday schedule with a weekend rotation and on-call shifts, managing a caseload across Burnsville area communities while delivering high-quality nursing care in patients' homes. You will complete OASIS assessments, develop care plans, and educate patients to promote independence and safe home settings.
