Job description
Phoenix Home Care is seeking a Home Health RN Case Manager in Kirkwood, MO. This full-time role features a Monday–Friday schedule in a home health setting, with a base pay range of $78,000–$88,000 plus potential earnings per visit and a $10,000 sign-on bonus.
You will oversee a caseload, supervise LPN visits, and coordinate with PT/OT/ST teams. Training is provided, and you’ll have autonomy to plan visits while maintaining quality patient care.
