Job description
Interim HealthCare is seeking a Registered Nurse (RN) Case Manager to join our home health team in Portsmouth, New Hampshire. The role offers a full-time, Monday–Friday schedule with independent patient visits and meaningful, compassionate care in patients' homes.
You will assess patients, develop care plans, coordinate with physicians and therapists, and educate families while ensuring quality and safety of care in a supportive environment.
