Job description
Lifespark is hiring a Home Health RN Case Manager for the Northeast Metro region. The role plans, directs, and ensures compliant home health services, with care planning, client/family engagement, and team coordination across the service area including Roseville and surrounding towns.
With 2+ years RN experience, you’ll lead assessments, develop plans, supervise aides, and maintain documentation while collaborating with physicians to optimize patient outcomes.
