Job description
Performance Home Health is seeking an experienced Registered Nurse Case Manager in the Dallas–Fort Worth area. This role oversees patient cases, coordinates care across the home health continuum, and ensures compliance with Plans of Care and physician orders.
Monday–Friday, primarily in-office with occasional home visits. The ideal candidate has 2+ years RN Case Management and 1+ year Home Health Case Management experience, with strong clinical judgment and communication skills.
