Job description
AccentCare, Inc. is seeking an RN Case Manager for Home Health in Chisago City, MN. The role is PRN with annual compensation estimated from the posted pay range. You will manage care plans from admission through discharge, coordinating across nursing and allied staff to ensure quality patient care.
