Job description
UR Medicine Home Care - Geneva seeks a Registered Nurse Case Manager to provide direct patient care and oversee case management in a patient- and family-centered setting. The RN case manager conducts initial assessments, plans and revises care, coordinates services, and educates patients and families.
Schedule: Monday–Friday, 8:00 AM–4:30 PM with on-call rotation 2–3 days a month and 3 holidays per year. Sign-on bonus up to $15,000; location serving Wayne, Seneca, Ontario & Yates Counties.
