Job description
Tufts Medicine Care at Home in Lowell, MA is seeking an experienced Home Health RN Case Manager to deliver comprehensive in-home nursing care. You will perform assessments, implement interventions, and coordinate with patients, families and the care team to promote safety and avoid unnecessary hospitalizations.
This full-time, days position requires an active RN license, valid driver's license, BLS (and CPR if hired after 9/28/2026), plus 1 year of acute care nursing experience.
