Job description
Nascentia Health is seeking an Adult Homecare RN Case Manager to deliver compassionate, in-home nursing across Onondaga, Oneida, Oswego, Jefferson and Cayuga Counties. This full-time role supports chronic and complex adult patients with autonomy and clinical decision-making.
You’ll coordinate care, educate patients and caregivers, and work with an interdisciplinary team. Competitive pay with a $15,000 sign-on, comprehensive benefits, and tuition reimbursement for BSN after six months.
