Job description
Lifespark is seeking an experienced Home Health RN Case Manager to plan, organize and direct Lifespark services in the North Central area. You will develop care plans, oversee start of care, coordinate with the care team and ensure regulatory compliance.
This full-time role offers a salary of $87,000–$90,000 annually, mileage reimbursement, and a schedule of Monday–Friday with on-call as needed, plus a comprehensive benefits package.
