Job description
FreedomCare is seeking a Case Manager, RN to serve patients in Ohio, conducting in-home assessments, coordinating services, and educating patients and PCAs. This field-based role requires frequent travel to patient homes in Youngstown, OH.
New graduates are welcome. The nurse will work with interdisciplinary teams to ensure safe, effective, person-centered care, maintaining regulatory compliance and high-quality outcomes.
