Job description
aah is seeking a Home Health Registered Nurse Case Manager to serve the Southeast Chicago area. Normal hours are Monday–Friday 8:00am–5:00pm with rotating weekends every 4th weekend and two holidays per year. The role includes after-hours on-call coverage and a sign-on bonus up to $3,000.
Responsibilities include building nurse-patient relationships, direct patient care, care planning, and ensuring quality, cost-effective care across the continuum. Requires RN license and current CPR.
