A legacy that will make your life better as well.As a Home Health RN Clinical Manager, you will manage a multidisciplinary team to provide outstanding patient care to achieve the highest quality outcomes. You will oversee patient care plans and ensure the appropriate care coordination between all disciplines, physicians, and caregivers.Some of your primary responsibilities will include:Auditing charts for clinical oversight, compliance, and adherence to the Medicare guidelinesManaging clinical activities, including patient assessments, care plan development, service level determination, and on-site field visitsDirectly supervising clinical team and participating in the recruitment, selection, and orientation of team membersEnsuring service quality and participating in care coordination to ensure proper communication between caregivers, patients, referral sources, etc.Discussing operational issues, updating staff on new/changed regulations, and reviewing records to ensure regulatory complianceOverseeing the assessment and reassessment of patients, including updating the care plans and interpreting patient needsCoordinating communications between team members, attending physicians, and caregivers to ensure appropriateness of care and outcome planningParticipating in performance improvement activities, maintaining ongoing clinical knowledge through internal/external training programsIdentifying clinical problem areas and documenting deficiency trends in a proactive approachBenefits You Will EnjoyIn addition to competitive pay, full-time team member benefits include:Paid Time OffInsurance benefits (medical, dental, vision, life, and other voluntary group programs)401 (K) retirement savings plan with employer matchTuition reimbursement for qualified career pathsCareer advancement opportunitiesScheduleFull TimeEducation, Licensure & Certification RequirementsActive Registered Nurse (RN) licenseMinimum of three (3) years of home care experienceMinimum of one (1) year of experience in clinical management
