Baptist Home Health Care by BAYADA is seeking a Registered Nurse (RN) to fill the position of Clinical Nurse Manager for our Jacksonville, FL Medicare Certified, Home Health office.

nThe Clinical Manager is responsible for overseeing daily clinical operations and ensuring that all home health nursing services comply with Medicare regulations, accreditation standards, and internal policies. This leadership role supports the development of staff, promotes adherence to best practices, and ensures optimal patient outcomes through data-driven performance improvement initiatives. They will lead and support a team of clinicians in delivering Medicare-certified home health services. The Nurse Manager is also responsible for ensuring regulatory compliance, promoting clinical excellence, and maintaining operational efficiency.
nResponsibilities for a Clinical Nurse Manager:
nnnProvide direct supervision and support to field RNs, LPNs, and clinical staff
n
nnEnsure compliance with Medicare Conditions of Participation (CoPs), state regulations, and agency protocols
n
nnMonitor and manage quality assurance processes, including OASIS documentation, Plan of Care development, and clinical chart reviews
n
nnFacilitate staff orientation, education, and performance evaluations
n
nnCollaborate with the interdisciplinary team to ensure coordinated, efficient, and patient-centered care
n
nnParticipate in the hiring, training, and retention of qualified clinical staff
n
nnSupport survey readiness and lead corrective action plans following audits or compliance reviews
n
nnTrack key performance indicators (KPIs) such as hospitalization rates, patient satisfaction, and documentation timeliness
n
nnProvide clinical support and case conference leadership as needed
n
nnLead regular case conferences and foster a culture of collaboration and accountability within the clinical team
n
n nQualifications for a Clinical Nurse Manager:
nnnRegistered Nurse (RN) with 2or more years of experience as an RN is required
n
nn2 or more years of experience in a leadership/ supervisory role.
n
nnExcellent organization and communication skills.
n
nnDesire to lead, inspire, and motivate others.
n
nnStrong knowledge of home health policies and procedures
n
nnDemonstrated ability to manage clinical operations and lead teams through change
n
