Job description
Sentara Home Care in Harrisonburg, VA is seeking an experienced RN to join as Home Health Coordinator. The role coordinates and supervises nursing care for participants, participates in interdisciplinary team meetings, and conducts initial and periodic assessments.
The ideal candidate holds an Associate's Degree in Nursing, an active VA RN license, BLS within 90 days, and at least 1 year of nursing experience (home health preferred).
