Pride Health is hiring a Home Health RN to support our client's healthcare facility in Elizabethtown, Kentucky .
Job Details
Schedule: Monday-Friday, 8:00 AM-4:30 PM EST (40 hours/week)
Location: Elizabethtown, KY 42701
Float / Coverage Area: Elizabethtown, KY
Job Type: Available for both Local and Travel
Contract Length: 13 Weeks with a high possibility of extension
Pay Range: $52/hr - $56/hr
Mileage Reimbursement Rate: 0.48/mile
*Pay offered is based on experience, expertise, credentialing, and education.
Duties
Complete OASIS assessments including Start of Care, Resumption of Care, Recertification, and Discharge visits.
Deliver skilled nursing care and ongoing patient assessments in the home setting.
Maintain accurate and timely clinical documentation.
Coordinate care with physicians, caregivers, and the interdisciplinary team.
Participate in on-call rotation and ensure high-quality patient care.
Requirements
Education: Associate Degree in Nursing (ADN) require
License(s): Active Kentucky RN License
Certification(s): BLS (AHA)
Years of experience: Minimum 1 year of RN experience
