Job description
Phoenix Home Care and Hospice is seeking a registered nurse for home health in the Kansas City area. The role focuses on building individualized care plans, coordinating with physicians and the health team to ensure high-quality patient outcomes.
You will assess patients, implement plans, and document care while collaborating with therapists and other team members. A Missouri license and driving are required, with flexible availability across full-time, part-time, and PRN shifts.
