Job description
Northwestern Medicine in Chicago, IL is seeking a Registered Nurse - Flex HHC to provide flexible home health and hospital-based care. The RN will assess patient status, coordinate care, and collaborate with the health team to develop and implement individualized plans of care across settings.
Required: BSN, current Illinois RN licensure, and BLS certification; strong interpersonal and analytical skills; commitment to equitable, high-quality care and patient education.
