Job Title: Home Health RN
Location: Grand Rapids, MI(coveringMuskegon)
Setting: Home Health
Pay Range:
Schedule: 08:00 - 17:00
Contract Length: 13 weeks
Candidate Type: Traveler or local candidated accepted
Requirements: 1 year of Medicare Certified Home Health experience. 1 year of HCHB EMR and OASIS experience
Responsibilities:
Provide comprehensive nursing care to home health patients.
Assess patient needs and develop individualized care plans.
Monitor patient progress and update healthcare team accordingly.
Educate patients and their families on health management and disease prevention.
Document patient care activities accurately and timely.
