Job description
Granite Peak Home Health is seeking a licensed registered nurse to provide comprehensive home health care aligned with the plan of care. You will perform initial evaluations and ongoing assessments, initiate and revise the plan of care, and deliver skilled nursing services in patients' homes.
You will coordinate services, counsel patients and families, participate in inservice programs, and maintain thorough documentation while adhering to agency policies.
