Job description
Advocate Health’s Home Health Nursing department in Charlotte is seeking a Registered Nurse to manage a patient caseload in the home setting and ensure care per physician orders and the established plan of care.
Responsibilities include data collection, coordinating a multidisciplinary team, performing treatments, and documenting progress while prioritizing high-risk patients to prevent hospitalizations. On-call and weekend coverage is required; sign-on bonus is available.
