Registered Nurse (RN ) – Full Time

Responsibilities
Provide skilled nursing visits in patients' homes
Assess patient conditions and develop care plans
Coordinate with physicians and interdisciplinary team members
Educate patients and families on disease management and care plans
Maintain accurate documentation
Qualifications
Active RN license in Arizona
Home health experience preferred (but not required)
Strong clinical and communication skills
Ability to manage a flexible field schedule
