Job description
Hartford HealthCare at Home seeks a Primary Care Nurse to deliver skilled nursing care to patients in home and community settings in Connecticut. You will follow care plans, document care, educate patients and families, and collaborate with physicians and care coordinators to achieve optimal outcomes.
Required: CT RN license, Associate degree or nursing diploma, and at least 1 year of nursing experience; home care experience preferred.
